Does anyone have a data conversion project plan sample I could use for an ERP data conversion

Did you look in the template section here? You may find a good starting point.

If it is for an ERP project, most ERP solution & consulting firms would have such boilerplates. Have you checked the online knowledge bases for the ERP solution provider you are focused on?

I'm with Kiron, usually the ERP providers have lots of different templates

The ERP provider did not have one - they rely on the customer to build one. I also checked the template section here and did not find a good one to use. I ended up building one myself. Ironically, I posted this question back in January of this year and the first responses came this month. : )
