Telecom Billing application

Deliverable acceptance criteria - less than 5 defects from Project Specification limits
SPI, CPI should be 1+/- 10%

Please help me in preparing Process Improvement Plan, Quality Metric and Quality checklist

That is not enough. Definition of Quality for that organization is critical to understand. Quality must be defined at strategical level. To define Quality you have to know who is the client and the value you are delivering plus the amount of grade that its willing to accept.

You also need to align with the quality policies of both your company and that of your client - those will strongly influence these deliverables.

For example, your client might have defined quality metrics and/or quality checklist templates which they'd like to see utilized for this project.
