Your preferred templates for managing notes and answers

My company currently utilizes confluence to store project information for easy access for employees.

I am looking for a better way to organize project notes and answers provided by the customer. I want to be able to keep track of when the notes were provided and by whom.

We have experienced situations where the customer provided an answer, then a few weeks later, their answer has changed. I want to make sure that when this happens, I can easily point back to the original answer and confirm what they truly want. This is just one example. In general, I prefer my notes to be well-organized but easy to locate.

I use OneNote for my personal notes but as an organization if you already have a licence for Confluence it's probably worth sticking to that. Is the issue that you can't view the audit trail for a page?

I'm looking for templates that others use or used that kept their notes organized. I have something now but I don't love it.

If your organization has SharePoint, you can create a form for data entry which will save back to a list. The list can be sorted, filtered, or exported as needed. SharePoint automatically tracks creator, modified by, and associated dates. So in your case, you can filter the list by stakeholder, or project name. It would also be possible to expose the form through an iframe in Confluence (potentially, I am not familiar with Confluence, but would assume it has widgets to enhance functionality).

If no SharePoint, use an excel file, but make sure it is centralized, and users update a single document with proper version control. What does your organization currently use as their system of record?

I and most of my peers use excel to track customer responses and other project details. There are a few who live and die by OneNote.

Sharepoint is great as a repository to upload documents, share, edit, modify files etc.

That's also one of my weakest points.
Usually I have an Excel log with the key points mentionning the e-mail date and where I have stored the info.

New ideas are welcome :)

But now I use Confluence tool (The best decision we ever made especially since we moved to JIRA for our SCRUM)

There are meeting notes templates in confluence- if that's what you meant.
