Successful project management is all about documenting project scope decisions. What is the problem in scope management? Project managers have not been exposed to best practices in documenting requirements. Project managers have not been trained in negotiating conflicting requirements. To make the project situation worse, many organizations don't have business analysts to assist the project manager in the specialized tools and training needed to elicit, specify, document and confirm stakeholder agreement with the requirements.
