Best way to present project issues to client

I have an issues list, that is more PM focused. I wanted to put the issues list in a manner that causes our client to take action, and really understand the issues and why it is important to resolve them. Does anyone have a good client focused template/sample for presenting issues. My goal is, once they look over the issues list, they will be able to resolve or at least discuss ways to resolve the issues that are at hand. The reason why the PM focused issues list will not work is our client is very much non-tech.

Susan - try the attached status at a glance. I've used this format in previous lives with success. The key to filling it out is keeping it simple. Top 5 issues, problems, activities, etc.
