A project is in the final stages of testing when it receives a major change request from the customer which would delay the final delivery of the project by 2 weeks. What should the Project Manager do NEXT?

a) Create a change request and go through the change management process
b) Evaluate the impact of the change
c) Communicate the implication of the change to the customer
d) Adjust the project management plan and baseline

The order for the process should be:

B - Evaluate
C - Communicate Impact

If Customer is fine with the implications then:

A - Create Change Request

If it was approved and depending on the extent of the change:

D - Adjust and update the documents to reflect the latest changes and keep the plans up to date.

Tricky question as I am not a PMP and still very new.

Assuming the change has already been evaluated of its impact, I would communicate the implication with the customer.

In my view the first action that Project Manager (PM) should take is

b) Evaluate the impact of the change

and then PM should
c) Communicate the implication of the change to the customer

Impact of the change needs to be evaluated and communicated to customer quickly as it may have impact on the delivery timelines.

PM shall complete the above steps urgently and seek Customer's consent to proceed with

a) Create a change request and go through the change management process
d) Adjust the project management plan and baseline

In my view, create a detailed plan on how the change request will impact the project and present it to client. If possible include cost of expediting the change and communicate it to the client.

Irrespective of the stage in which the project is, you need to follow the change management process. Initiate the change process. Once you follow the change management management process, rest of the things like analyzing the change, approval or rejection of change, rebaselining of plan will follow....

Based on the phrasing of the question, I'm going to assume there has been some initial evaluation of the impact. "would delay the final delivery of the project by 2 weeks"

A quick communication of the implication of the change may be needed ahead of starting the formal change management process. In particular if delivery is expected next week, initiating a change request without communicating the expected two week delay would practically be "withholding information".

From an agile perspective, if the needed change is high urgency/importance, the change management process may need to be expedited.

If the product/result will not deliver the needed value, or worse yet be in violation of some law or create a hazard, then somebody needs to be putting on the brakes right now and figuring out next steps...
