What Agile maturity assessment models/tools have you used?

There seems to be an increasing variety of Agile maturity models and assessment tools. What are some that you have used or know about?

The two that I have been involved with were both CUSTOM, based on specific aspects of the organization.

Yes Kev, the majority seem to be customized models. I was hoping there would be a proven model that many find useful in multiple applications, but there really doesn't seem to be one that is widely used.

Yes Kev, the majority seem to be customized models. I was hoping there would be a proven model that many find useful in multiple applications, but there really doesn't seem to be one that is widely used.
