Auuugh! I hate it when I'm wrong, but I love the fact that you guys are challenging me.
Yes, PMBOK 6th edition says a change request is a formal proposal to modify any document, deliverable or baseline. It also says that "Change requests" also include corrective actions, preventive actions, and defect repairs. In my experience, we have never treated defect repairs as formal change requests, but as we all know, the test isn't based on MY experience! :-)
That said, I have to amend my answer to agree with Kiron. I don't like any of the four but answer B is closest to correct. I had read something into answer C that is not there. C is incorrect because changes are APPROVED in this process so the PM hasn't yet approved them if we're just beginning to consider them. Kiron for the win.
