The only, major pitfalls that I see are
1. A copy-paste culture, where people copy without due diligence or proper review and modifications
2. No standard WBS or any other document can cover all situations, so you have to think out of the box and add what could be missing and is unique to your project. Saving Changes...

Any standardized structure creates on major pitfall, we use it like a checklist. And we forget to check what is different, what doesn't fit in the standard Saving Changes...
