In early days of agile, there was very high failure rate probably due to lack of experience and work cultural issues.

The 2015 Chaos report from Standish did show a higher percentage of successful projects which were delivered using agile lifecycles & methods than those following a traditional approach.
However, a core cause for failure of agile delivery lifecycles persist, namely the lack of adoption of an agile mindset on the part of the delivery teams and supporting stakeholders.
