In one of my projects, one business VP (who managed operations and finance) sponsored an IT project. Since all consultants were external, he was the client and he/ his department was the one who was end customer for that product.

Sure - when you contract a general contractor to build a deck in your backyard, you are being the sponsor, client & customer all in one for that project...

Often. I am in local government. The agency sponsoring us is also our customer most of the time.
