In IT world there are a lot of rotation in the organizations, in the last few years I observed that organizations care, and managers understand the issue and care too. Human Resources departments are doing a huge effort to keep the talented employees and retain the knowledge.

Whaaat!!! I just deleted the comment! I was answering Sergio while reading while having lunch in front of my computer, then I finished reading, I realized that he made his point and opinion about knowledge transfer, then I close the browser, went to a meeting and when I'll come back...Bang! it was published.

Sorry Sergio if you saw my initial comment, wasn't my intention!

Employers believe that everyone is replaceable which is true, but the cost of someone to leave is much higher than they think.
Unfortunately, I'm agree with you. But I thing this our responsibility as project management professionals to addressing this subject in our community.

Top managers should always have a plan for knowledge management and encourage the same. Employees may also be leaving because of better opportunities or just to be able to do something different. Within the same company, you might just stagnate since you know your way around. Top managers who truly care about the employee may not offer resistance so that the employee can follow their aspirations. This also keeps the door open should the employee want to return in future.
