If you search the "genesis" of Agile you will find that it was born in 1990 inside the USA DoD/NSF Agility Forum created inside the Leihigh University. The forum took 3 years long where the top companies in the world (located in USA) defined what Agile and agility is finding an alternative of Lean. One of the pillars of Agile to gain into agility is Knowledge Management. So, all this comment is intended to reinforce your point about lost knowledge. It is implsible to implement Agile, it is impossible to gain into agility without maintaining knowledge. That is by definition of Agile and agility
To find some answers to this critical question I'd recommend you read two books:
Daniel Pink - Drive
Patrick Lencioni - Three Signs of a Miserable Job
If leadership teams would do a better job of addressing the issues identified by those two authors, there would be much less job dissatisfaction and premature attrition.

Whaaat!!! I just deleted the comment! I was answering Sergio while reading while having lunch in front of my computer, then I finished reading, I realized that he made his point and opinion about knowledge transfer, then I close the browser, went to a meeting and when I'll come back...Bang! it was published.
Sorry Sergio if you saw my initial comment, wasn't my intention!
