Sounds like a blocker list to me so not sure what the tie in to scheduling is?

Excel works for the former well, but so could any number of record-based tools including MS Access, JIRA & SharePoint.

If you want to directly associate blockers with activities on your project schedule, then if you are using MS Project, you could capture these in a custom text field at the activity-level...

While you could search in the templates section of this community, if you are looking for a way to associate issues with specific activities in your schedule, as I suggested, all you will need to do is add a custom text field in MS Project (e.g. Text1, Text2...), rename it, and use that to capture either the details of the issue or an issue ID which could then reference your (separate) Risks/Actions/Issues/Decisions log...

Sounds similar to a Risks and Issues list. Can you repurpose one of the many spreadsheets that are available? What data are you not able to capture and why?

All i wanted to have a list similar to you have mentioned"Risk and Issues List". Cannot capture the outcomes of the issue as the tasks are inter-related.

Interesting. Would a mind map, fishbone or similar diagram help visualize the relationships?
