The Drive-by Praise!!! That one drives me crazy.
Praise you for something minor and ignoring a major accomplishment is another one.
And here is one a previous manager of mine did. We would be in a full team meeting, say 25 people. She would go around and praise each person for some accomplishment they did. But then there were a few people that she was not sure what to praise so they would get the generic "and X has been doing some great things for her team".
Of course, not having your back is a demotivater.
And telling you that you just do not understand all of the things that are happening in the department. (talking down to you).

Dinah, thanks for your contribution and showing support to the points, I agree with you it is also important to let the team feel that their performance in line with strategic plan of the organization.
1. Purely recognize individuals
2. Treat them like mushrooms
3. Behave inconsistently
4. Don't walk your talk
5. Play negative office politics

Kiron, thanks for your good points, I like that example of mushroom though I am not a fan of eating mushroom LOL :-) it is like Pseudo-Analysis or Blind Development, running of a company where the communication channels between the managers and the employees do not work properly, it is very important to keep every one informed and have good communication channels mostly I found our-self like sandwich between team and higher management.
The most common terrible management approach I've seen is "Do better or you're fired!"
This fails to address what the employee did or didn't do correctly. Even worse, redirects the employee's focus from the task at hand - into self-survival mode, in which the employee is preoccupied with fear of losing employment. This is likely to only bring about more errors in work completed.
The second most common phrase which demotivates anyone working on a project are the words "I don't care what it takes, just get it done!"
This tells employees, their causes for concern or any existing roadblocks in their tasks are not important.
This brings about the attitude of "malicious compliance" in staff. Anything not explicitly stated in the work assignment will not be carried out, even if the employee can see it is required. Work completed will likely not be documented properly either, as staff rush to complete their assignments as quickly as possible to get out of the project.

Conrad, totally agree with you that theory of X is almost died and the quality of work under fear is never be like other organization having highly motivated team and they treat the work like their own. Thanks for your contribution.
