Project scope must be defined from product scope. Usually what change is product scope not project scope. That is very important to take into account. Relating to changes in the project the only way you will survive is by creating a project change mangement process, publish it to all the project stakeholders and follow it.

How to prepare the project scope management process? Is there any specific tool?

Three key tools to leverage are:

1. Baselines - that's what is approved
2. Change management plan - that tells you how to manage changes to any of the project's baselines
3. Project change requests - the most common artifact for formalizing changes

Freeze the Project Scope by getting a formal sign off of project plan. Any change to the scope from that point onwards will nned to necessarily follow a predefined change request process.

Note that a change in scope at an initial stage is preferable than when done half way through the project.

Welcome to the project management world! Try to avoid the scope creep. Change management plan and related documents are the best source.

Creating it following the project scope knowledge area inside the PMBOK if you are following the PMI way to manage projects. You have to tale a process model which is outside the PMBOK (not matter you can find a detailed reference inside the new PMBOK related to life cycle process) first.

Try to collect

Requirement management plan
Change management plan
And also look at the enterprises environmental factors and organizational process assets.

Make sure more changes can lead to Scope creep and it should not be happen many times in any project.
