Business case exists into the PMBOK before 6th edition but in the actual edition lot of things related to documentation have been reviewed to integrate the business analyst role. Business analyst is in charge of all related to business case creation. The important thing because there is not an standard about the business case document itself is the spirit: business case is the document where the right people will say "yes, I will put resources on this initiative". So, no matter the name organizations give to the document, no matter the format organizations use, there must be a document or something similar with the enough information to convince the right people that the initiative deserves to be started. The initiative is the solution to a business problem where solution is equal to "the thing" to be created (product/service/result) plus "the process" to create it (project). Saving Changes...

I have come across many cases of projects without a documented business case, even in environments that require/enforce business cases for projects. This has usually come about for two reasons 1] It started as an operational initiative that grew and morphed into a project; 2] to Eric's point, It was an idea that was proposed by someone quite senior in the organisation that was adopted/mandated for realisation.
