I consider this a rhetorical question. Someone has to lead the project, whether internal or external, and take responsibility to get the job done. Anyone who can fit that bill is good enough to be the 'professional' PM.

yes, you right. but the point is PMI has its own requirements to award the PMP. Moreover, the employers may ask you to have your PMP certificate to be eligible for the role.
It depends. If it is a cross-functional project of significant complexity, often someone with more of a project management focused skillset will be utilized since there is a lot of "adminis-trivia" involved. On others where one the project is primarily related to one technical discipline and of limited scope, a SME in that discipline who has solid technical skills can be more efficient. Saving Changes...
Depends on the organization's criteria for what is deemed a project and what level of complexity requires a "titled" PM.
For example, with my current client, they have a full OpEx group staffed with LSSMBBs and BBs, and the BBs are usually running DMAIC projects themselves. If the follow-up work (e.g. a technology project) from a DMAIC project is large & complex, a "titled" PM is assigned.
I don't see the distinction. There are project managers for internal improvement projects just like any other project. It's a project, and its managed, the label is irrelevant :-) Saving Changes...
