I use an eight question survey in SurveyMonkey every two sprints for the teams I'm coaching. With a free account, I can send it to my full team but can't export the results so I copy & paste them into Excel.
Like Rami say listening to the team member should give you a good portrait.
A survey is an interesting option if you make it blind.
