Besides good communication skills and leadership qualities, I think one of the main characteristic of a proactive manager is that they identify risk at the beginning of the project and then manages and monitors risk throughout the project.

Perfecto!! Thinking ahead, looking at the big picture as well as the details...noting down the challenges and taking steps ahead...all these are part of proactive project management...last but not the least...communicating ahead, and not waiting until client follows up with us.
I think part of the nature of project management is proactivity, so it should be a given. Saving Changes...

Few of the characteristics of a proactive project manager, that I can think are
1. Keep a tab of the risks / costs - "proactively" ascertain if these are going to be affected.
2. Have a complete idea of the flow of information and deliveries to create the final product / deliverable. keep revisiting these on the board to be completely in control (and of course, checking the other stakeholders to see if any of these are getting affected)
3. Keep a check on the external / internal dependencies and "proactively" reminding the "owners" of these dependencies, as well as "What gets affected in terms of delivery and schedule", if these are not met
4. Leadership skills - be "proactive" and keep talking to your teams to get an insight in to the issues they are facing OR if they have ideas for the betterment (which I am sure they would have multiple).
5. Maintain an Action register - update it very regularly and share with all "action owners" Saving Changes...

In addition to other replies, I believe a Proactive PM must have:
1. Ability to forecast future with certain level of accuracy.
2. Better understanding of Organisational Internal & External Environment
3. Authority & Command on his team
4. Better understanding of Organisational Processes.

Agree with Sergio and Sante - Project management is about being proactive. If you think about it most of what you do is proactive. You preempt what needs to happen by defining a schedule with milestones, you identify possible risks and risk management strategy should any of the risks be realized...... Saving Changes...
