What major tailoring of PMI framework /Processes /PMBOK have been adopted in your organisation to align it to your Organisation's requirements and Objectives.
For example: In our organisation there is no Project Charter to authorize the Project, Instead a Project Appraisal Document, basically a kind of Business Case is used to authorize & approve a Project.
Most mature organizations will have a PM method based on the PMBOK or some other well recognized framework. This will usually define both mandatory and discretionary practices and supporting templates. Detailed tailoring is left to project teams to do based on the context of their project.
In other organizations where there are no standards, it is usually left to whoever is leading the project and the project team to define how much or how little they will align with any particular PM framework.

Thanks Kiron , i appreciate your response but my aim of initiating this discussion is to exactly see what are those particular changes and deviations adopted by different organizations around the world and how one can recognize and differentiate. I mean not general discussion , but particular instances, if one can describe.
Terminology is not important. There is always a tradeoff between organizational culture and PMBOK culture. You only need to balance the situation. Saving Changes...

A simple example of this is the RAID log which is a common artifact defined in many company's methods. This consolidates the essence of a Risk Register, Assumptions Log and Issues Log which are defined as outputs/inputs in the PMBOK Guide.
Instead of a project charter, some companies use the approval of a project intake form as the official authorization of a project's existence.

my (new) employer has adopted PMI/PMBoK Guide (and some other methods, principles like PRINCE2 ... ) and created a complete 7 layer framework around all that).
It is like Krion already said, any company has to adopt those principles they want to use for their work/standards and tailor them in a way that they should fit into all the existing processes and workflows (or they need to align them too).
Since my company does mainly customer projects a specific example is, that we do not have a process "develop project charter" (like yours too); but we do have a process called "develop a proposal" instead.
I could mention tons of more examples like that, but this would be far too much for a posting here.
Developing that Framework (it is called PRIMER) was a major project itself and has won the PMIs "project of the year" award several years ago.
