Need a survey template for BI/dashboard user experience

I am interning for a manufacturing company this summer. The company implemented a BI/dashboard initiative six months ago and wants to know how satisfied users are with the dashboards (are the dashboard helping users do their jobs better? are users using the dashboards as inteded? are users satisfied with the chosen KPIs?, etc.)

I'd like to find a survey template that focuses on BI user exprience feedback. If anybody has a lead, please let me know.
