I'm completing a PMP Exam Prep class, which was a great kick start to my studying, but there is something missing that I cannot find. I am looking for a case study where ALL of the PMI process outputs are documented with examples. It would really help solidify the whole thing for me if I could see how it all lays out and connects. Does anyone know where I can find this type of information?

I think the best way I can help you is to point you a nice forum where people taking the PMP Exam share their lessons learned about the Exam and the preparation for it. There's a lot of value there

A yahoo group called "PMP Best" also exists - you may find other resources there. You do have to become a Yahoo! member.

One of the best books I read that provided real examples mapped to all the knowledge areas of the PMBOK is the following book by Kathy Schwalbe which is not even a PMP prep book, but should be read prior or during your preparation:
In addition, the book's support website provides templates, case studies and white papers to further illustrate the real world applications of a framework like the PMBOK. I highly recommend the book.
