Same as 2017 (and 2016, 2015...) - lack of predictability around people allocation, whether that was Product Owners or team members.

A co-worker suddenly left and I had to take over one week before a major program milestone review. I had no notes to go on regarding prior work, contact list, etc. I was simply thrown into the deep end of the pool and had to swim while everyone watched.
