As an expert in the field of project management, you probably have experienced, seen or heard of fail projects story. Based on your experience, what are the 3 to 5 main project failure reasons? Saving Changes...
1) Design changes
2) Lack of Communication
3) Lack of Authority (Constantly overruled, lack of support, delays in funding of previously approved financing by senior management)

The answers here are just as interesting as the question! Here're a few additional reasons projects misfire -
Improper communication
Poor resource management
Reluctant buy-in
Measuring the wrong things...or not measuring at all!
Drawing narrower cost estimates and then ending up overrunning them without the results to show for it.
Unrealistic goals and timelines which pushes your teams to work too hard.
What's important to note here is that it happens to everyone, so the solution isn't to curse your luck but to rather, inspect your processes more closely and get feedback from your teams as to what's preventable and what constitutes reasonable targets. And of course, never hurts to explore a different project management methodology! Saving Changes...
