I don't think there is a certain form for this. You probably can create your own based on Domains vs. Percentage.

I hope you did the examination and got through. Udemy has a few exam simulators as well for PBA Examination. I did the PBA Examination early this year and got through. I did one of the simulators before sitting for the exam Saving Changes...

Did you notice that PMI changed the experience qualifications for their PMI exams, including the PMI-PBA from # of hours to # of months? This not only simplifies the application process, but actually makes it easier to qualify. For example, if you worked as a hybrid PM/BA on a project between Jan 2020 - March 2020, that's 3 months of business analysis work experience regardless of how many of those hours were actually spent on business analysis work. Curious what everyone's thoughts are on that?

Frankly speaking, not very impressed with this change. PMI should have made the application process more rigorous in order to ensure those who really qualify only sit for the exam.
