Is there a deliverable called risk response plan?

Is there a deliverable called risk response plan?
Which document should we refer to find out the risk response to be implemented?

In my personal experience we use the risk register to put the response plan inside it. Obviously the risk response plan could generate other documents and in fact a parallel project too.

As per PMBOK, the risk register should contain risk response list

Basic risk responses are captured in the risk register but the "footprint" of a risk response might be seen in many other PM documents including the WBS & schedule or budget.
