Biggest risk ever taken in my project management experience was to do an ERP re-implementation project without any base documentation in an organization which had 60%of its process customized in the product to support their unique business scenario.
Method adopted to minimize this risk was to run a systematic change management committee to convince stakeholders to adopt standard business processes and in parallel building a perfect business blue print for each business process through continuous interaction with stakeholders.

Biggest risk in project is not following proper methodology. As it may leads to cost and schedule over run and has to compromise in quality.

From my side, hire weak subcontractors instead of main pre-qualified contractor to execute work. It's the biggest risk and impacts of time and cost!!!!

Taking on a 2-digit million, 3 years, fixed price project with 11 pages of statement of work.
Biggest risk I have ever taken on was agreeing to "fix" a turn-around project. The previous PM had quit with no handover. The project was 3.2 times over budget and required safety critical control systems with an imminent non-negotiable due date. My team succeeded in delivering it on time, safely and without any additional cost. It was a reputation risk, but thankfully went well Mashallah. I didn't sleep well during though Shadav!

Trying to manage high uncertainty projects with a waterfall approach. At least, hybrid must be used in these cases.

The biggest risk present on our Project is HSE related. Since its an 400kV Overhead Transmission line Construction project of 100 km in extremely hilly terrains with 209 nos. of Tower and we already had 02 nos. of fatalities last year.
