What are the tools mostly used for improving the process activities?

Conversations. Good old fashion dialogue.

Other tools that can help, mind mapping and diagramming - then possible to feed into a workflow system if so desired. Regardless, having the process diagrammed or mind mapped will help in itself.

Interesting enough, we had a meeting today where it was mentioned that we needed to create yet another "process". However, it was mentioned by our leader, that in itself this may not be the total solution. Instead, and as Andrew stated, sometimes problems can be solved faster and have better results just by reaching out to that someone, having a conversation over a coffee, and building that relationship.
