I would like to get people's input on how they control project charters and scope documents. Do project managers control them locally? So they go through a formal document management process?
Does anyone assign these documents part numbers and control them with an ECO process?

I would expect the answer is, it depends. If the project is for government work you can bet a very formal ECO process for change control and strict scope management.
My experience has been I control the scope and change management via monthly project meetings where we go over current state and any proposed changes that have been submitted.
Submitting a proposed change is as easy as sending an email with details that I can cut and paste into the table showing all of the proposals and if they were accepted.
