In my organization we have the both areas and I'm having difficulties to have a good aproach with them. Basically becacuse there aint enough resources to cover the whole project, never the less, I think is fundamental to be in the same page. The most likely reason is because I haven't finish to understand the reach of change management. I was in charge to create the process we use in my actual work place. I will assume that you are talking about organizational change management. In our case, while they are roles, we have three main roles into each initiative: business analyst, project manager, and change manager. Sound dangerous to subsidize BA to the implementor. You let the implementor decide on what you need and what Oracle will install and charge you!

Well ! we are running organisations which have shrinking margins so they cant afford to have dedicated employees for each role , So to be competitive employees need to wear multiple Hats and making things Happen , so coming back to your question of Change Management according to me is Making changes to Legacy Applications / configurations in Application / Network refresh / Digital Transformation - Key learning would be study of existing systems at least functionality wise and conducting workshops with the current platform Subject matter experts and understand the platform and then a followup workshop on how they will look at the change and the guidance they can provide (which is very crucial for your success) and then it would be easy working with the Change management Team. In my case, the company im working for is starting to adopt change management activities. So you can imagine there are a lot of opportunities in that matter. I think the project Im managing has a good resource of CHM (change mgmt) but I'd like to improve the comunication between us because it's crucial to be on the same page in order to approach all the implementation team.
The change management I'm talking about is the area/resources in charge of cultural change and communicating in a way that all the project/organization understand what we are trying to achive, also, to help the users change their mind set in order to comply with the organization goals, etc. Saving Changes...
