Stephane, I love the fact that you understood the question...
I also love the answer, because even though it is not stated in the PMBOK as a primary output, I would also choose "Perform Integrated Change Control" because it is constantly referenced to that process.
examples:
PMBOK Page 131. Change log. Described in Section 4.6.3.3. The change log contains the status of all change requests.
PMBOK Page 120. 4.6.3.3 PROJECT DOCUMENTS UPDATES
Any formally controlled project document may be changed as a result of this process. A project document that is normally updated as a result of this process is the change log. The change log is used to document changes that occur during a project.

We can only call a request a "Change Request" after the approval of "Project Management Plan". So first item (first change request) can be listed on "Change Log" after its submission. This can only be in "Perform Integrated Change Control" process.
How do you go with this logic?

Think the 'change log' is a change control tool (4.6.2.2). A change request is tracked thru it, as it is registered, assessed, approved, tracked and communicated (think implementation and quality control is missing in this list).
The relevant inputs/outputs are the change requests themselves. Saving Changes...

If you are managing a project with an agile approach, a change would be considered a pivot. Since agile is client/customer centric, the change could be recorded in a "lessons learned" log. Or listed in the updated back log.
There isn't a formal change process in agile, so I'm just guessing about this. Saving Changes...
