Listen to the water cooler gossip and pay close attention to stakeholders who seem to be wanting more frequent updates. Both are a sign that there might be a perception of trouble even if there isn't really.

I had a team that tried crowd-sourcing input. It didn't work at all.

Maybe it works in other organizational cultures but whether it's management by walking around or a more structured means to gather information, actively seeking out input on the state of things seems to work a whole lot better than expecting people to voluntarily offer up how things are going.
