While I don't have an example of a sign-off sheet you might meet with less resistance for signatures if you reshape the sign-off to be an authorization to proceed to production testing or implementation; whatever the case might be.
The form doesn't have to be fancy. Just a declaration that the application has been tested through Beta and that to the best of their knowledge believe it is ready for the next stage... eg. authorizing the project to go forward.
For some sign-off represents acceptance without recourse and that meets with much resistance.
