Key to define an approach is to understand that project is the mean to create a solution because solution is equal to "the thing" to be created (product/service/result) plus "the way" to create it (project). The approach must be decided as part of the organizational analysis that is made before the project exists (as part of business case creation activities) where business analyst is accountable for that. To decide the approach the environment (internal and external) and "the thing" to be created must be taken into account. Some project managers fail along the years because they put focus on process instead of put focus on creating the product/service/result as defined (scope and quality) in the time needed and with the estimated costs. But the focus is the product and all needed must be defined with focus on the product. On the other side you always have a plan. More or less explicit but you always have a plan.
