How often do you expand the scope of project to incorporate new deliverables?

Sometimes deliverables shift and scope needs to be expanded. How often do you expand the scope of existing/approved project? Every other project? Every 100th project? Never?

How often? As much as my stakeholders need and they approved the change request.

If only a project came wrapped with a neatly tied bow...

The important thing for the PM is to not react emotionally to requests. The PM needs to ensure he/she understands the change, that the exec sponsor agrees with it, then assess whether there is a cost/schedule/tradeoff impact. Remember to include the time to assess the impact in any preliminary estimate.

Again the PM needs to be rational and calm through the process.
