There are many paid assessment tools out there, Sadia - for example, CEB has a set of assessments targeting key stakeholders needed for improved delivery outcomes (https://www.cebglobal.com/information-tech...-skills.html/), but these are at cost.

You could also take a framework such as PMI's Competency Development Framework (https://www.pmi.org/pmbok-guide-standards/...nt-3rd-edition) and use that as a starting point.

I created a leadership assessment tool for my webinar, a few years ago. It is based on Toastmasters' leadership competences model, as presented in the High Performance Leadership project..

You might find it a good example for what you want to do.
