No project must fail if you: 1-before everything start you define the metrics and meassures to evaluate if the project can be considered successful or not. 2-you perform project management which is to anticipate to events by managing risk and issues mainly. 3-you perform governance (or as you want to call it) meetings to evaluate if the project must be kill, put on hold, continue with base into the project performance information you are accountable (as project manager) to publish. I had project failures. They were caused by weak, inept and vacillating leadership. The only way to deal with that is to change jobs. The place I am describing still hasn't integrated a small software package that should have taken 3 months. It's now close to 4 years. Saving Changes...
1) Acknowledge the failure.
2) Figure out what you can learn from the failure.
3) Apply what you have learned to other projects.
4) Help others understand what you learned from the failure and why it matters.
If you don't do #4, the first three don't matter. I learned that the hard way. Saving Changes...
