Helping built a PMO in support of a customer

Need a good baseline to assist a customer in standing up a PMO organization. This organization will be a mix of customer and contract personnel.
Can anyone recommend good references documents or books that provide a general "How To" guide, which include some means to measure effectiveness.
Looking for tools outside the normal PMBOK libraries.
