Most critical sign for project failure?

There are quite a few candidates in my mind.
Members request for quitting the team
Unachievable schedule
Strongly over budget
Poor relationships with customer

To me, the team member requesting to leave seems the worse one. Not just impacting the current project, probably impacting the future too.

One of the fundamental laws of the universe is "cause and effect" which unfortunately many today do not subscribe to. What you seem to be describing are the effects of some underlying causes. You mentioned what I would refer to as secondary effects for people wanting to leave such as over budget, unrealistic schedule, poor customer relationships, but you then need to examine what were the cause(s) for those effects. Fundamentally, it would seem poor project planning is the root cause of what you are dealing with.

Project Sponsor apathy is huge. You could have sterling execution but if your sponsor doesn't care about the project it'll die a slow agonizing death.
