One of the fundamental laws of the universe is "cause and effect" which unfortunately many today do not subscribe to. What you seem to be describing are the effects of some underlying causes. You mentioned what I would refer to as secondary effects for people wanting to leave such as over budget, unrealistic schedule, poor customer relationships, but you then need to examine what were the cause(s) for those effects.
