Good Topic - One of the roadblocks I've encountered is adapting the organization and achieving buy-in for the Agile Mindset / Agility.

The organisation can be (shoud be) agile if they employ agile techniques or frameworks or not. Organizational Agility is more related to sustainability, survival and resilience and is a known area since 2000+ years (Panta Rhei by Heraclitus).
BTW project management is not necessarily agile or sustainable. Sometimes it is just about solving a problem quickly no matter what (comes next). To fight a threat, relieve from pain, enable progress on greater goals.

I have worked in organisations where teams are silo'd and blinkered to other departments. This brings many challenges when trying to deliver projects across the business.

In addition to other's comments, buy-in from leadership and a true understanding of what it means to undertake an Agile transformation and truly understand the intent, goal, and vision. If this fundamental piece is missing, then cannot expect others to know the direction to go.
