Set up a tracker in excel to capture the data; Probability, Impact, etc.. Then set up the visual using a scatter diagram with your x and y-axis depicting probability and impact levels (low to high) respectfully. To help highlight the levels of each, make the background of the chart a diagonal gradient color from green to red. Once complete, snip an image of the chart, pop into PPT. Saving Changes...
