A charter is usually at a high level and if no baselines have been established then you wouldn't go through ICC. If there is a major shift in direction or purpose for the project then you may need to confirm that there is still willingness on the part of the sponsor and key stakeholders to continue to invest in the project. As such, the charter may need to be updated and re-approved.

I had that exact thing happen to me. We had negotiated a specific budget for the project which went into the project charter. The project charter got approved. It was then decided to change the project budget. Only after the change request was approved did I go back and change the project charter and get it re-approved. Saving Changes...
When this question has come up before, the general consensus seems to be that the charter is not revised. It certainly can be, but in practice that is rare. In the past I have been a change management SME for a very company with extremely comprehensive processes. I think I have seen change incorporation processes defined for just about every type of document we produced except a charter.
Managing a portfolio of smaller projects, what I have seen when there is a significant change in scope such as there is a decision to merge two projects together in order to capitalize on the synergies between them is that projects may be cancelled, and a new project generated to combine them. Fundamentally, that really is no different than revising a charter, but it is a complete departure from the intent of the original projects rather than scope creep. Saving Changes...

Somewhere I read that Project Charter is the birth certificate of a project; so it never gets changed like the birth certificate of a person.
Project Charter is developed by sponsor and the sponsor is the owner of it? If it gets changed, who makes the changes? Project manager or Sponsor?
Another question is what are the circumstances when PC gets changed? Saving Changes...

The purpose of a charter is to initiate. Initiation occurs once. So the charter is not updated. Some decisions are made and hold forever. I like the comparison to a birth certificate.
The process 'Create Charter' may occur several times in a project though. For example when you initiate a phase in the context of a phase gate review. The output would not be a project charter but the appropriate documentation and approval for that phase.
Having said this, any organization builds their own standards, based on PMBoK or not. I also have seen many organizations confusing the process groups with phases (and then wondering why the phases do not reflect product reality).

Not at all. Project Charter is before the project exists as an entity. When changes happend then all related to project will change, if apply, starting from the project scope but not from the Project Charter. Saving Changes...

Even birth certificates have to be changed when you legally change your name.
