We have a government agency in every city (Standesamt) which handles this, e.g. in case I change my name because of marriage they document the marriage and any name change. They also have access to a record of my birth status from the city where I was born.
The passport issuing agency will check with their data base too.

There are a few exceptions when a German birth certificate can indeed be changed, by a civil court. For example if the husband of the mother at birth (who’s name you get as birthname) is not the real father, and this is found out later, you may apply to the court to change your birthname to your real father‘s. The answer to the original question has to be in consideration of the intent and assumptions identified in the Project Charter itself. That being said, the Charter has to be at a high enough level not to require revisiting every time the wind blows. However, any project documentation that is not subject to review and adjustment to reflect evolving conditions becomes a liability rather than an asset. It is no longer a project benefit enhancing tool but a risk. "We are doing it this way because the Charter says so regardless of the circumstances!" A good project manager will ultimately ignore the Charter if it does not suit his/her purpose and do what's right for the project (beg forgiveness later if necessary). Applying the 'Birth Certificate' analogy the only remaining option is a 'Death Certificate'. Saving Changes...

On some Instances Charter has to be changed, else it is not reflective of the changes of High level requirements, Estimated Project Expense , Statement, risk or timeline.There are examples where even the name of Project has to be changed to meet some conditions or statuary requirements.
