From these options, only b is a plan and a project document.
a. is a PMBoK process
c and d are activities

If you look at PMBoK 5.1.3.2 you will find the wording of the question describing the requirements management plan

Per your suggestion, I looked at PMBoK 5.1.3.2 and found out that your answer indeed should be the correct answer. Thank you for helping me - specially explaining why other choices can't be correct.

I have a very similar opinion to that of Kiron: "This looks like a sample PMP exam question so should get moved into the Certification Central group."

I wrote my exam in 2003. I don't remember being able to rule out answers based on whether they were worded as an activity or as an input/output.

I sat at the exam in 1997 with 400 questions to be answered in 8 hours. Was on exam item writing sessions after that several times. There was a session to assure each question is unambiguous.

Since 1999 till today I am giving PMP Prep classes, which forced me to deep dive in every PMBoK edition.

Expect fair questions but consider all logical arguments.

From the list of answers you presented, I agree with Thomas but normally, this is all available under "Change Management Plan". At least we have one of each project. Just be mindful of this.
