Project Team Feedback/Survey

I'm looking to use a questionnaire that can be distributed to the members of a project team at completion of the project to get their feedback on the project (ie. was communication with the team adequate, did they feel like they were involved in decision making, etc.) Does anyone have a sample of one that has been used on your projects? I'm looking for specific questions rather than general open ended questions like "what went well on the project".
