User Acceptance Testing can be confirmed by email as well, but unless the System goes live the Business will reluctantly sign-off, they need to see that everything is working well and must also include sign-off by IT department who must accept responsibility for maintaining and supporting the system.
Device 2 simple forms:
1. Detail the system implemented etc and acceptance sign-off by the business
2. Similar details for the IT to take ownership of the system

Acceptance after UAT is the system works according to specification,
agreeement when the outstanding bugs will be fixed,
Furthermore for Business and IT it is important to have the documentation, user guide, operational guide, installation guide etc, is complete and up to date.
Training material is in order for business and IT
From an IT perspective after the UAT there are often tests to make sure the application runs smoothly in th technical environment.
In case you host the system at a third party or have a third party facility management party in house again acceptance criteria will be defined
So it depends on who is the O&M manager and what is his responsibility.
