Risk Management - tracking and assigning

Hi,

I am looking to implement a friendly-user method to track and asign risks issued from internal risk meeting into a word/excel file and linked to outlook. I would also like to use the document to track any meeting notes so we can follow each item during a project at one central location. If there would be an easy way of assining a resource on each issue to outlook or anything else, that would be great.. Anybody could share their files?
