I do not agree about quality will be "suboptimal" because time and cost constraints. Quality will be as defined taking into account time and cost. But what is usually forgotten is grade. In accordance to the strategy you have to define the balance between quality and grade.

Yes . My point is many times the organisations adopting agile are not that matured and do not get needed support from leadership to follow agile discipline that leads to situation where scope gets tailored for managing cost and the defect fixation is not that rigorously observed. I agree with you avoiding defects at first place.

It's true that results can suffer if agile ways of working don't get adequate sponsorship and support within an organization. I work in technology and it seems that too often agile transformation is something driven by IT teams rather than being championed by the whole of business management and executive leadership.

Delivering the real needs of the customer sounds like an issue of prioritisation rather than quality. Presumably the customer gets to decide on the priorities and that ought to include any defect fixes. Avoiding the defects in the first place was the topic of my previous answer in this thread.

In general the statment from you is true. But this will also depend on the organization culture and learning implementations.
If we want to improve there are ways to improve better quality, by keeping the constraints.
